Freshman Recommendation Forms

First-year applicants: High school students are required to submit one Counselor Recommendation form. Use the link below to connect to this form. Please print the form and give it to the proper individual. Please allow references at least two weeks notice to send in your recommendations. We also encourage you to provide your references with an addressed, stamped envelope to help ensure prompt delivery.

Berry College Counselor Recommendation Form

Home School Students: Home school students are required to submit two personal recommendation forms in lieu of the counselor recommendation form required of other freshman applicants with a high school guidance counselor. Use the link below to connect to this form. Please print these forms and give to the proper individuals. Please allow references at least two weeks notice to send in your recommendations. We also encourage you to provide your references with an addressed, stamped envelope to help ensure prompt delivery.
