Berry College requests that incoming students submit all required health form requirements using their Medicat Patient Portal prior to attending SOAR. Within the Patient Portal, each student must complete a Medical History form, a TB Screening Questionnaire, and upload an official Immunization Record.
STUDENT REQUIREMENTS
- Medical History Form (self-reported and completed in the patient portal)
- Immunization Record signed by a Healthcare Provider OR an official Department of Health record
- TB Screening Questionnaire (if screening indicates, a TB skin test or lab test)
IMMUNIZATION REQUIREMENTS
- Tetanus (TDAP) vaccine within the last 10 years
- Two Measles, Mumps, and Rubella (MMR) vaccinations
- Meningococcal conjugate (MCV4) vaccine within the last 5 years OR a signed waiver stating that you understand this requirement but decline the vaccine. This is required by Georgia state law for all residential students.
