Berry College requests that incoming graduate students submit all required health documents using the Medicat Patient Portal prior to the start of their graduate program. Each student must submit a self-reported medical history and tuberculosis screening questionnaire, official documentation of required immunizations, and a copy of their medical insurance card, if insured.
STUDENT REQUIREMENTS
- Medical History Form (self-reported and completed in the patient portal)
- Immunization Record signed by a Healthcare Provider OR an official Department of Health record
- TB Screening Questionnaire (if screening indicates, a TB skin test or lab test)
- Medical Insurance Card (copy front and back).
IMMUNIZATION REQUIREMENTS
- Tetanus (TDAP) vaccine within the last 10 years
- Two Measles, Mumps, and Rubella (MMR) vaccinations
